Hello I look forward to working with you. Here are the steps to make an appointment (these steps can be done in a fairly brief amount of time): 1. This current page provides information for you to read now. 2. The bottom of this page links you to a page of information (privacy policies, etc.) all health professionals are required to give to patients. You can look at that at your convenience when you have time. 3. The bottom of this page also links you to a secure page with a questionnaire to fill out insurance information. I must receive this completed questionnaire before our first session. I appreciate your participation in following these steps. It will allow more time in the session for me to help you. The webpage you are now reading provides: A. Instructions for if your insurance provider requires preauthorization. If you need preauthorization, or have to find out if you need preauthorization, and you cannot call your insurer now while you are online reading this, I suggest you print this page as a reference for when you do call them. B. Instructions for if your insurance provider requires a physician's prescription for treatment. C. Basic information for you to know before we meet, including emergency contact information. Information to know when contacting your insurance provider or physician:Your insurance provider's behavioral health company:Many health insurance companies have a separate Behavioral Health company. Please look on your card to see if this is true for your company. If you are not sure, call your Insurance provider. When you go to the website questionnaire, you will be asked to enter the name and phone number of your provider's Behavioral Health company, if applicable. Preauthorization #: If this treatment is to be submitted to your health insurance, call your insurance provider to ask if preauthorization is required. Most companies give preauthorization right away on the phone. Cigna HMO does not require preauthorization. Cigna PPO does require preauthorization. If your company (such as United Healthcare) has both a medical and behavioral department, call the behavioral department (# on the back of your card). If they need a description of the treatment, below are descriptions to tell them: If primarily for sleep difficulties, inform the insurer this is: "Outpatient psychological treatment for relief from anxiety and sleep difficulties, with a primary diagnosis of 300.02, Generalized Anxiety, and a secondary diagnosis usually 307.42, Insomnia Due To Generalized Anxiety." (For your information, the reason for this diagnosis is because: A. Insurers need a diagnosis. B. Insurance companies' behavioral departments cannot utilize a primary diagnosis of insomnia, as that would shift you into the medical department which would cause insurance coverage complications. C. Anxiety is always a factor, so this is an accurate diagnosis. D. Anxiety is the mildest diagnosis available, so when it truthfully applies it is the best to use.) If primarily for pain, inform the insurer this is: "Outpatient psychological treatment for relief from anxiety and pain, with a primary diagnosis of 300.02, Generalized Anxiety and a secondary diagnosis usually 307.89 Pain Disorder Associated With Both Psychological Factors and a General Medical Condition." (For the reason for that diagnosis, see the previous paragraph.) If primarily for anxiety or depression, inform the insurer this is: "Outpatient psychological treatment for relief from anxiety or depression, with the primary diagnosis being 300.02, Generalized Anxiety". (If, after meeting with you, I determine that a different diagnosis would be appropriate, I will discuss that with you.) If you have been working with another therapist and already have a diagnostic code that has been submitted to your insurance company, please inform me and we can discuss utilizing the same code, if it is appropriate for our work. Therapeutic Treatment with Richard Shane, Ph.D. - Policies(Part A - Please read this now)Fees: $130 per session. If you are utilizing insurance, your fee will just be your co-payment. The fee is to be paid at the time of the session. With the Sleep Easily treatment, you will receive two CDs, and written materials. The fee for these materials is $32. For phone consultation, there is no charge for the first 10 minutes. Longer therapy calls are prorated at the session rate. Cancellations, missed appointments, lateness: You are responsible for 50% of the session fee for any missed appointment or any cancellations of less than 24 hours notice, extreme emergencies or weather excepted. If you arrive late, we will still have to end at the previously scheduled time. If you have a really bad cold, please postpone your session, as the cold may make it difficult to learn the sleep breath. CD player: If you are wanting to resolve sleep difficulties, one element of the treatment is a CD of my voice guiding you through the procedure. You listen to this while falling asleep, so please have a CD player in your bedroom by the day of our first session. If you don't currently have a CD player, these can be purchased inexpensively at places like Target or Radio Shack. Think of whether you would rather listen to the sleep CD through speakers or headphones (both ways are effective) and get that type of player. You soon are able to also use the procedure without the recording. Caffeine: If you drink caffeinated beverages, please have a minimum of caffeine in your system at the time of our session, as this will make it easier for you to have a deeper experience. If an emergency should arise when I am unavailable, call the Boulder County Mental Health Center Emergency Psychiatric Service at 303-447-1665. During my vacations or absences from my practice, I will designate a backup therapist to cover any emergencies.
