Secure Horizons: Secure Horizons always requires preauthorization. If your do not receive preauthorization, I will work with you for the $80/ session rate that Medicare reimburses.

If your insurance provider is not on the above list and you want to find out if they might cover this treatment, call the person who processes insurance for my practice, Kristi Hunter at 303-699-5585. You are ultimately responsible for the bill. However, we do everything to help insure that the insurance company will pay.

If preauthorization is required, call your insurance provider. Healthcare companies that require preauthorization: United HealthCare, Cigna PPO, auto insurance carriers and worker's compensation. Tell your insurance provider this is three-session out-patient psychotherapy for sleep difficulties due to anxiety or medical conditions, CPT code 90801. If you are working with other insurance companies, it is your responsibility to find out if they require preauthorization. If you need help with any of this, call the person who processes insurance for my practice, Kristi Hunter at 303-699-5585.

Physician's prescription: If your sleep difficulties, anxiety or pain were caused by an auto accident or work injury and this treatment is to be submitted to your auto insurance carrier or worker's compensation, you need to have your referring physician fax to me (as soon as possible) a prescription for treatment - or your physician could give you the prescription and you could fax it to me. My fax # is 720-294-8350. If you physician is not already familiar with my treatment program and needs more information, contact me and I will send that information to your physician.

Regarding preauthorization: Prior to your first visit, it is your responsibility to find out if preauthorization is required by your particular insurance plan—or you will be billed for the charges. If you have any problems, please call the person who processes insurance for my practice, Kristi Hunter at 303-699-5585.

  • Please review the information health professionals are required to give to patients. At the bottom of this page is the link to that information.
  • Please complete the insurance form. At the bottom of this page is the link to that secure page.

CD or mp3 player: One element of the Sleep Easily treatment is a CD of my voice guiding you through the simple steps. You listen to this while falling asleep or back to sleep, so please have a CD player by your bedside to use the night after our first session. If you don't currently have a CD player, you can purchase one inexpensively. It is easier to listen through a speaker so you don't have to deal with headphones. Most partners who sleep reasonably well find the recording comforting in the background. If you have an mp3 player, I will email you mp3 files specially labeled for that.

Additional materials you will receive: In addition to the Sleep CD, you will receive a SleepGuide Manual and other materials. There is a one-time charge of $15 for these materials.

Cancellations, missed appointments, lateness: You are responsible for 50% of the session fee for any missed appointment or any cancellations of less than 24 hours notice, extreme emergencies or weather excepted.

Caffeine: If you drink caffeinated beverages, please have a minimum of caffeine in your system at the time of our session, as this will make it easier for you to have a deeper experience.

To schedule your sessions: After you have completed the basic information questionnaire (which you will be linked to below), call my office, 303-449-0778. (You might want to write down this number to make it easier to call after you have submitted the questionnaire.)
